Policy Agreement

By submitting this application, I acknowledge and agree to the following terms and conditions for participation in the Spay and Neuter Program:

1. Eligibility: I confirm that the animals listed in this application meet the eligibility requirements, including being a companion animal (dog, cat, or other), and that I am the legal guardian of these animals.

2. Proof of Low Income: I confirm that I have submitted valid proof of low income, as requested by the program, and understand that this is a requirement to qualify for reduced spay/neuter services.

3. Spay/Neuter Procedure: I agree that the animals listed will undergo the spay/neuter procedure as part of this program. I understand that the surgery will be performed by a licensed veterinarian, and that it is my responsibility to follow any post-surgery care instructions.

4. Program Participation & Appointment Requirement: I understand that participation in this program does not guarantee immediate availability for services. Once my application is approved by RESCUES ON85TH, I must schedule an appointment directly with Alabama Spay & Neuter Clinic. I acknowledge that RESCUES ON85TH will place my name on an approved list for payment at Alabama Spay & Neuter Clinic. If my application is not approved, payment will not be made on my behalf, and my cat(s) and/or dog(s) will not receive services under this program.

5. Photographic Documentation: I agree to allow the Alabama Spay and Neuter Program to take and use photos of my animals before and after the spay/neuter procedure, including a photograph of the animal at the time of the procedure. Additionally, I must take a picture with my cat or dog while holding a sign that reads: “I got spayed and/or neutered with Rescues on 85th.” This photo is required for my application to be processed. These photos may be used for promotional purposes, educational materials, or reporting, and I consent to such use without compensation.

6. Liability: I understand that the organization and affiliated parties (including veterinary partners) are not responsible for any complications, injuries, or damages arising from the procedure, and I agree to hold them harmless from such occurrences.

7. No Refunds: I acknowledge that there will be no refunds for the spay/neuter services, as they are covered by RESCUES ON85TH through the Alabama Spay & Neuter Clinic.

8. Compliance with Guidelines: I agree to comply with all program guidelines, including the timely submission of required forms, supporting documents, and scheduling my appointment with Alabama Spay & Neuter Clinic upon approval.
